About the DOT Physical

As a certified medical examiner, Dr. Moya Martin provides comprehensive DOT physical exams
for commercial drivers to ensure you meet Federal Motor Carrier Safety Administration (FMCSA) standards.
When you qualify, you receive your Medical Examiner’s Certificate (DOT medical card) the same day.

What to expect
  • Government-issued photo ID (driver’s license, passport, etc.)
  • Medication list with dosages and prescribing providers
  • Wear/bring your glasses, contacts, or hearing aids if you use them
  • Take your usual medications unless your treating clinician advised otherwise
  • Drink water before your visit (a urine sample is required)
Bring These If They Apply to You
  • Vision exam report from an optometrist/ophthalmologist (if corrective lenses or known vision issues)
  • CPAP compliance report for sleep apnea (typically last 90 days)
  • Most recent A1C (for Type 1 or Type 2 diabetes)
  • Cardiologist clearance letter for history of heart disease, stent, bypass, heart attack, arrhythmias, etc.
  • Neurologist letter for history of stroke, seizure, or brain tumor
  • Gastroenterologist letter for significant digestive disorders (e.g., GI bleeding, severe liver disease)
  • Endocrinology or primary-care summary for thyroid/adrenal disorders if recently unstable
  • Specialist notes for any condition that could affect safe driving (e.g., pulmonary, renal)
  • Prior DOT medical card and any related determination letters, if available
  • Glasses, contacts, hearing aids you use while driving
